Stuck in systems infancy
The unofficial definition of insanity — continuing to do the same thing but expecting different results — is widely known. In my work world, this notion is recognized by a much larger percentage of people than those who can name all seven quality tools or explain the difference between common cause and special cause variation. The irony is that we are headed down the insanity path if we don’t change our management ways, yet we hesitate to challenge prevailing management systems that suboptimize the performance of our value streams.
